POSITION SUMMARY
Provides community resources and short term follow-up to individuals initially seeking services, from office based and/or community setting.
ESSENTIAL DUTIES AND RESPONSIBILITIES
The essential functions include, but are not limited to the following:
• Provides community linkages for those not eligible for services, including referrals to external resources and follow up to ensure they get connected.
• Assist with Medicaid Presumptive Eligibility application and monitor referrals using outside portal.
• Follow up with clients regarding need to or return to Open Access or missed initial assessments.
• Provides back up coverage as needed for intake coordinators including initial screenings and scheduling.
• Travel between agency locations to support Open Access including initial screenings and scheduling.
• Review and track guardianship packets to make sure they are completed before client is seen for intake appointment.
• Assist medication services staff by providing external referrals/resource information.
• Maintains knowledge of admission criteria per agency protocol and insurance coverage.
• Maintains broad knowledge of community resources.
• Maintains constructive working relationships with community collaterals.
• Maintains Avatar knowledge and experience.
• Other duties and/or responsibilities may be assigned by supervisor.
MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)
Education/Certifications:
• Bachelor’s degree in a related field required.
• Active driver’s license and registered automobile available for use during work with minimum state-required auto-insurance
• CPR and First Aid Certification Required
Experience Required:
• 1 - 3 years minimum position related experience, previous case management experience preferred
Required Knowledge:
• Prior mental health and/or substance use knowledge required.
• Knowledge of community resources and willingness to research as needed.
• Intermediate to Advanced knowledge of Microsoft Office Suite (Word, Excel, PowerPoint, Outlook and Publisher).
Skills/Abilities:
• Strong crisis management skills
• Excellent communication skills, both written and verbal.
• The ability to use and troubleshoot basic business equipment.
• The ability to interact with a wide range of individuals.
• Strong ability to multi-task with a high level of accuracy and timeliness in project output.
• The ability to maintain a high level of client confidentiality.
• Ability to problem solve quickly and prioritize tasks for optimum efficiency.
