Description
POSITION SUMMARY 
Provides psycho-social/educational groups and community activities to adults with Mental Illness.

ESSENTIAL DUTIES AND RESPONSIBILITIES
The essential functions include, but are not limited to the following:
• Facilitates psycho-educational groups to educate and assist consumers with coping skills, socialization, health management and development of adult daily living skills.
• Oversees community-based activities and outings with consumers.
• Assists with program development, i.e. creating new groups and activities, coordinating special events and outings, and soliciting for ticket donations.
• Assists with transporting consumers to and from the program as well as to various community destinations.
• Completes necessary documentation, including weekly PSR progress notes, case management notes, discharge summaries, etc.
• Provides support to consumers by providing face-to-face case management services, networking with other providers, and referring to outside agencies and community resources.
• Provides direction, support and resources to consumers to help them grow toward greater independence, autonomy and empowerment.
• Provides outreach services to consumers when they had been absent from the program.
• Oversees and ensures that PSR program meets accreditation requirements by maintaining food, kitchen and day room safety checklists
• Maintains required training and certifications.
• Maintains knowledge of agency technology programs and/or systems needed to do the job and uses them effectively.
• Ensures consumers know their rights and responsibilities and are treated with dignity and respect.
• Other duties and/or responsibilities may be assigned by supervisor.


MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)

Education/Certifications:
• Bachelor’s degree required, preferably in psychology, social services, or similar. 
• CPR/First Aid Certification required
• Valid Driver’s license and minimum state-required vehicle insurance required
• Class E driver’s license required
• Reliable transportation required

Experience Required:
• No previous experience required, however 1 to 2 years prior similar experience preferred.

Required Knowledge: 
• Knowledgeable of general behavioral health diagnosis and familiarity with trauma-informed care practices preferred.
• Knowledgeable of HIPAA rules and how to protect client confidentiality.
• Basic computer knowledge and the ability to use Microsoft Outlook.  

Skills/Abilities:
• Able to maintain an engaged, positive attitude in an ever-changing environment.
• Good customer services skills and an ability to communicate verbally and in writing with a wide variety of people.
• Effective in crisis situations, locating and coordinating services to assist consumers in crisis.
