Description

Position Summary

 

In cooperation with the assigned school district, provides substance use services to students within the assigned school district. The position serves as an active team member of a School Based Trauma Informed Care Team to address student and family needs and functioning within the academic environment and community.

 

Essential Duties and Responsibilities

The essential functions include, but are not limited to the following:

 

  • Provides community based services to assigned caseload, utilizing independent judgment and decision-making regarding appropriate treatment needed.

  • Provides individual assistance to clients in accessing needed substance use and mental health services including accompanying individuals to appointments to address needs and assist with resource acquisition.
  • Provides assessment, evaluation, education, treatment, and case management to substance using persons in individual and group formats.
  • Assists clients with developing personal support systems, including, but not limited to, work with family members, legal guardians, recovery communities, etc.

  • Provides harm reduction counseling, overdose prevention and naloxone trainings to clients and their friends and families.

  • Provides Crisis intervention to consumer’s experiencing a psychiatric or substance use related crisis and utilize clinical judgment in recommending appropriate treatment recommendations, including the need for hospitalization or residential treatment.

  • Completes progress notes in AVATAR within 72 hours of service provided which clearly reflect interventions provided and necessity of those interventions.

  • Problem solves issues related to housing and housing crisis, accessing community resources to assist with basic needs as well as emergency situations, advocate for consumers in regards to receiving proper health care, and assist consumers in resolving matters of significance.

  • Uses a strengths-based and trauma-informed approach to work collaboratively with clients and treatment team members to help support clients in their treatment and recovery.

  • Provides group counseling and psycho-education groups as necessary.

  • Provides active outreach to persons who may be reluctant to engage and begin to develop a working alliance towards understanding their individual concerns and needs.

  • Maintains knowledge of agency technology programs and/or systems needed to do the job and uses them effectively.

  • Maintains required training and certifications.

  • Maintains required agency productivity standards (4 hours daily).

  • Attends and actively participates in all staff meetings.

  • Maintains a valid driver’s license, auto insurance, and reliable transportation

  • Other duties and/or responsibilities may be assigned by supervisor.

     

Minimum Qualifications (Knowledge, Skills, and Abilities)

Education/Certifications:

  • Associates Degree or Bachelor’s degree in the Social Services field or 4 years prior related work- experience required.

  • At least one year experience in working with clients with substance use disorders.

  • Prefer CRADC certification or equivalent

  • Active driver’s license and minimum state-required auto-insurance.

  • CPR/First Aid Certification required

  • Annual TB testing required

 

Experience Required:

  • 4 years minimum work-related experience required if no degree.

  • If degreed, 1 to 3 years prior work-related experience preferred.

 

Required Knowledge:

  • Knowledge of behavioral health diagnosis and symptoms.
  • General knowledge of community resources and/or the willingness to research as needed
  • Considerable knowledge of substance use disorders and DSM-5 disorders.

  • Knowledge of utilizing assessments in treatment planning for case management.

  • Knowledge of community referring agencies and collaborative strategies.

 

Skills/Abilities:

  • Strong crisis management skills
  • Excellent communication skills, both written and verbal.
  • The ability to use and troubleshoot basic business equipment.
  • The ability to interact with a wide range of individuals.
  • Strong ability to multi-task with a high level of accuracy and timeliness in project output.
  • The ability to maintain a high level of client confidentiality.
  • Ability to problem solve quickly and prioritize tasks for optimum efficiency.
