The primary responsibility of the Nurse Care Manager is to assist Tri-County Mental Health Services with the integration in daily clinical operations of the DMH Healthcare Home disease management initiative. The Nurse Care Manager will be an active member of the Health Team which will manage the full array of physical health needs, in addition to behavioral health care needs, and long-term community care services and supports, social services and family services for consumers enrolled in our Healthcare Home (HCH). To ensure optimal utilization of the program, the Nurse Care Manager will provide encouragement, wellness training and health information tool (HIT) training to the staff at Tri-County. The Nurse Care Manager will promote an enhanced culture of behavioral and physical health wellness within the organization.



  • Performs job duties in accordance with the Tri-County’s vision, mission, and values.

  • Maintains a caseload of consumers with chronic health conditions and serious behavioral health conditions and/or substance use disorders. Supports the “whole person” approach of the healthcare home program.

  • Champions healthy lifestyle changes, including tobacco prevention and cessation, nutrition and physical activity through education and support. Promotes preventive health care through evidence-based recommendations and education.

  • Facilitates connection to healthcare services and community support as needed.

  • Develops appropriate care coordination connections with primary care providers in the community, including Swope Health Services - Maple Woods.

  • Performs an annual health screening to assess health and medication history, health goals, risk factors related to suicidality, violence and other risky behaviors, knowledge gaps, personal strengths, needs, and barriers.

  • Tracks required assessments and screenings for each consumer, in accordance with performance measures, and follows up as needed.

  • Implements a person-centered treatment plan for consumers when needed. This will include the consumer’s present health goals and will be current, measurable and relevant

  • Coordinates with CS Workers about identified health conditions and the wellness coaching needs of their consumers and provides educational training on physical health conditions and medication adherence.

  • Participates in the Integrated Care Team (ICT) meetings.

  • For the DMH Net technology programs and initiatives listed previously:

    • Monitors HIT tools and reports for treatment and medication alerts, hospital admissions/discharges, and ER visits.

    • Coordinates with consumers, CS Workers and Care Team members regarding treatment and medication alerts and hospital admissions/discharges.

    • Performs post-hospitalization medication reconciliation.

    • Provides ongoing training and technical assistance to CS Workers on use of CyberAccess and the access of a Patient Profile Report.

  • Completes monthly Relias and other mandatory competency-based trainings that include, but are not limited to, Consumer Rights, Confidentiality, Consumer Safety, Employee Safety, and Therapeutic Boundaries.

  • Educates consumers regarding MyStrength technology as an increased support and education tool.

  • Meets the needs of persons served through prompt response to phone calls within 24 hours, flexible scheduling times and locations of appointments, and same or next day appointments.

  • Assists in developing and enhancing wellness initiatives such as, but not limited to, wellness support groups, nutrition and physical activity programs, periodic staff and consumer education classes, and consumer health fairs.


  • Associates Degree or Technical School w/RN certification required
  • Bachelor's degree preferred
  • Missouri RN license
  • CPR/First Aid Certification required
  • Active Driver's license w/state required minimum insurance required
  • Annual TB screening required
Experience Requirements:
  • 3 to 5 years of position-related experience minimum
  • 5+ years of position-related experience preferred
  • Medical/Surgical nursing plus psychiatric nursing experience preferred
  • Healthcare technology software experience preferred
Skills and Abilities Required:
  • Medical assessment skills, terminology, diagnoses and medication knowlege required
  • The ability to develop educational trainings and programs on various topics to promote and atmosphere of wellness required
  • Ability to deliver presentations to groups of people required
