Description
POSITION SUMMARY 
Provides outreach and case management to identified individuals needing mental health services. Provides crisis stabilization and assists with coordination of care for identified at-risk youth. 

ESSENTIAL DUTIES AND RESPONSIBILITIES
The essential functions include, but are not limited to the following:
• Provides community based services to high risk youth utilizing independent judgment and decision-making regarding appropriate treatment needed.

• Provides active outreach to youth and families who may be reluctant to engage in services and begin to develop a working alliance towards understanding their individual concerns and needs.

• Provides crisis intervention to youth experiencing a psychiatric or substance related crisis and utilize professional judgment in determining intervention needed including the need for hospitalization or residential treatment.

• Completes initial phone calls and follow up contact to consumers/families. 

• Provides follow up to hospitals and families regarding consumers who have been identified as high risk.

 • Collaborates with treatment team and is active participant in implementing the Zero Suicide initiative at TCMHS.

• Completes progress notes in AVATAR within 72 hours of service provided which clearly reflect interventions provided and necessity of those interventions.

• As needed, develops person-centered treatment plans initially and annually, which are strengths based, reflecting consumer’s life goals and individualized choices.

• Problem solves independently issues related to safety, finding community resources to assist with basic needs as well as emergency situations, advocate for consumers in regards to receiving proper health care, and be available to problem-solve matters of significance. 

• Maintains knowledge of agency technology programs and/or systems needed to do the job and uses them effectively.

• Maintains required training and certifications.

• Maintains required agency productivity standards.

• Attends and participate in weekly team meetings and monthly all staff meetings.

• Other duties and/or responsibilities may be assigned by supervisor.


MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)
Education/Certifications:        
• Bachelor’s Degree in a related field required
• Valid Driver’s License and state-mandated auto insurance
• Annual TB test required
• CPR/First Aid Certification required

Experience Required:              
• 1 to 3 years’ experience in a related field required

Required Knowledge:              
• Knowledge of youth behavioral health diagnosis and symptoms.  
• Knowledge of community resources and crisis management. 
• Strong understanding of Trauma Informed Care.

Skills/Abilities:                          
• Ability to build rapport with youth consumers struggling to engage in services due to mental health symptoms.    
• Ability to make clinical decisions independently. 
• Working knowledge of Microsoft Office programs, including Microsoft Outlook, and the ability to use the internet for basic tasks. 
• Ability to use basic business equipment.
