Description
POSITION SUMMARY 
The HIM Clerk-Data Entry is responsible for diagnosis entry, reviewing new chart documentation and determining if policy requirements have been met, scanning documents, and other chart management responsibilities. This position is also cross-trained to provide coverage for other Department positions as needed for absences.

ESSENTIAL DUTIES AND RESPONSIBILITIES
The essential functions include, but are not limited to the following:
• Enters diagnoses from Doctors/APRNs into EMR and update program information as applicable. 
• Reviews new chart documentation to ensure that it is complete. Notify responsible staff member if corrections are needed. Scan into EMR system.
• Reviews guardianship and delegation of parental authority paperwork and enter appropriate documentation into the EMR system.
• Opens new clients for treatment court programs into the EMR system.
• Reviews monthly discharge reports, notify clinicians of need to complete discharge summaries, and discharge clients in EMR per agency procedures.
• Scans Labs/UAs into into the EMR system.
• Liaisons with Treatment Court Coordinator during chart audits to ensure entry of all clients.
• Reponds to walk-in customers requesting assistance and log information into the EMR as appropriate. 
• Scans various types of documentation received into EMR as needed.
• Assists with PRN document requests throughout the day.
• Takes department phone calls as needed.
• Assists and provides backup and assistance for other HIM positions as needed/assigned.
• Reviews HIM procedures at least annually to determine if any updates/changes are needed.
• Other duties and/or responsibilities as assigned by Manager.

MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)
Education:
High School diploma or equivalent required; Associates Degree in HIM related field preferred

Experience Required: 1-2 years of similar position-related experience. One year of previous medical office experience preferred. One year prior experience working in a behavioral health environment preferred. 

Required Knowledge: Basic PC knowledge/skills with experience in Microsoft Office products required; Medical records terminology required; Data entry experience required; HIPAA/Privacy regulations required.

Skills/Abilities: Good attention to detail; the ability to maintain confidentiality; the flexibility to manage a variable workload; and the ability to interact with the Agency’s customers in a professional and courteous manner.
