SUMMARY OF POSITION:


Provides psycho-social/educational groups and community activities for adults with Severe Mental Illness.


PRIMARY RESPONSIBILITIES:
 
  • Assist in development of ITP, Behavioral Health Assessments and documents clients’ progress in PSR through weekly progress notes plus quarterly reviews, case management documentation and other documentation as needed e.g. critical incident reports.
  • Maintains required training and certifications.
  • Maintains knowledge of agency technology programs and/or systems needed to do the job and uses them effectively.
  • Assists with program development, i.e. creating new groups, coordinating special events and outings, and writing letters for tickets.
  • Developing weekly calendar of events; updating PSR sign in sheets and PSR worker case load list; completing weekly billing and copying attendance sheets to PSR workers for charting purposes.
  • Oversees and insures that PSR program meets accreditation requirements by maintaining food, kitchen and day room safety checklists
  • Provides or arranges transportation service to clients as needed.
  • Manages psycho-educational groups to assist clients in socialization, health management, and development of adult daily living skills.
  • Manages outreach services to clients who have been absent from day program, inviting them to come back.
  • Provides face to face case management to clients as needed or refers clients to community support workers.
  • Helps clients network with other clinical and support services as needed.
  • Effective in crisis situations, locating and coordinating services to resolve client crisis.
  • Provides direction and resources needed by clients to help them grow toward empowerment.
  • Insures that all clients know their rights and responsibilities and are treated with dignity and respect.
  • Encourages and assists clients in the development of community integration skills through management of community activities.
  • Other duties and/or responsibilities may be assigned by supervisor.

QUALIFICATIONS:
Education:  Bachelor's degree required
Certifications:  CPR/First Aid required or obtained within first 90 days of employment

                       Class E Drivers License
Experience:  None required but 1 to 2 years preferred
