Description

Position Summary

Provides crisis assessment and crisis case management services to high risk persons diagnosed with serious mental health issues and/or substance use. Acts as a liaison with area hospitals, internal staff and providers and outside agencies to ensure clients have face-to-face follow-up after hospitalization and/or resolution of a crisis. Provides mobile crisis response to area hospitals and participates in crisis back up rotation.

 

Essential Duties and Responsibilities

The essential functions include, but are not limited to the following:

  • Provides crisis assessment and crisis case management services to high risk population to ensure timely follow up after resolution of a crisis. Interventions include linking clients to TCMHS internal and provider network as well as community resources and providing face-to-face follow up as needed.
  • Acts as a liaison with community hospitals to ensure proper care coordination for clients upon discharge from hospital.
  • Acts as a member of the Mobile Crisis Response Team including mobile response to area hospitals.
  • Serves as crisis back-up clinician for crisis calls and crisis intakes as needed.
  • Tracks data outcomes in accordance with agency policy.
  • Completes all paperwork in adherence to agency timelines.
  • Demonstrates competent usage in agency electronic medical record,
  • Assists the Crisis Manager in identifying service gaps and steps to address service needs.

Minimum Qualifications (Knowledge, Skills, and Abilities)

Education:
Master’s Degree in social work, psychology or related field.

Must have valid driver’s license, state required insurance and reliable transportation.
CPR/First Aid Certification and Annual TB testing required.
 

Experience Required:
3 to 5 years of similar position-related experience

 

Required Knowledge:
Knowledge in managing behavioral health crisis.
Knowledge of Trauma Informed Care.

Skills/Abilities:
Strong assessment and diagnostic skills. Excellent ability to build rapport with consumers.
Ability to make clinical decisions independently.

Working knowledge of Microsoft Office programs, including Microsoft Outlook, and the ability to use the internet for basic tasks.
Ability to use basic business equipment.
